In recent weeks, I have received significant levels of correspondence with regards to Hampshire & Isle of Wight Sustainability and Transformation Plan.
Hampshire and the Isle of Wight is one of 44 areas across England that are developing detailed local transformation and sustainability plans (STPs) to find ways of meeting the opportunities and challenges facing the local health and care system. This has focused on both the need to empower people to stay well and also on how to provide safe, high quality, consistent and affordable health and care to everyone.
The plan does not replace or slow down local transformation programmes. Instead, organisations have come together to do the things that can only be achieved by working in partnership. This includes describing the implementation of a number of critical national programmes, including the Five Year Forward View for mental health and for general practice
The full STP Delivery Plan and a summary are now available on Fareham & Gosport CCG's website, here. This includes details on how you can get involved.
